Completed Fix the "Name:" section on Moderator applications

This is a small suggestion. On Moderator forms right at the start there is a question that asks for a name. However, when the form is submitted, the forum post shows an additional "Username" part above.
For example:

Thanks for the suggestion. Unfortunately Xenforo requires a placeholder for any type of form we create (i.e. mod applications). So there's no real way for me to delete the field as its mandatory.
